C-1 SUBMISSION OF CHARTER SCHOOL APPLICATIONS WITH RECOMMENDATIONS FOR APPROVAL AND AUTHORIZATION TO NEGOTIATE CONTRACTUAL AGREEMENTS FOR THE 2000-2001 SCHOOL YEAR

RECOMMENDED: That The School Board of Miami-Dade County, Florida:

1. approve ASPIRA South Youth Leadership Academy application and authorize the negotiation of a five-year charter school contractual agreement for the 2000-2001 through 2004-2005 school years;
2. approve The Pinecrest Preparatory Academy application and authorize the negotiation of a five-year charter school contractual agreement for the 2000-2001 through 2004-2005 school years;
3. approve Rosa Parks Community School/Overtown application and authorize the negotiation of a five-year charter school contractual agreement for the 2000-2001 through 2004-2005 school years; and
4. authorize the Superintendent to negotiate with the approved applicants the charter school contractual agreements which reflect the content of the applications, as approved by the School Board, and in compliance with Florida Statutes and State Board of Education Rule, such charter school contractual agreements to be submitted for School Board consideration at a subsequent meeting at which time there will be a public hearing to provide for community input.

C-2 AUTHORIZATION FOR THE SUPERINTENDENT TO ENTER INTO A CONTRACTUAL AGREEMENT WITH THE SCHOOL BOARD OF PACE BRANTLEY HALL PRIVATE SCHOOL OF SEMINOLE, FLORIDA, FOR THE SALE OF ONE PROJECT VICTORY LABORATORY PROGRAM

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent to enter into a contractual agreement with The School Board of PACE Brantley Hall Private School of Seminole County, Florida, for the sale of one Project VICTORY Laboratory Program, for the sum of $5,000, including training and related services, provided by Miami-Dade County Public Schools.

C-11 REQUEST FOR AUTHORIZATION FOR THE SUPERINTENDENT TO AMEND A CONTRACTUAL AGREEMENT WITH COST MANAGEMENT SERVICES (CMS), INC., FOR MEDICAID LOSS PREVENTION AND CLAIMING SERVICES, CHANGING THE STRUCTURE FROM A CONTINGENCY FEE TO A FIXED MONTHLY FEE, FOR A TOTAL AMOUNT NOT TO EXCEED $66,767, FROM FEBRUARY 10, 2000 THROUGH JUNE 30, 2000

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent to amend a contractual agreement with Cost Management Services (CMS), Inc., for Medicaid loss prevention and claiming services, changing the structure from a contingency fee to a fixed monthly fee, for a total amount not to exceed $66,767, from February 10, 2000 through June 30, 2000. The appropriation for this item is included in the General Fund of the 1999-2000 Adopted Budget.

C-12 CONFIRM A CONTRACTUAL AGREEMENT WITH THE SCHOOL BOARD OF HILLSBOROUGH COUNTY, FLORIDA, FOR MEDICAID ADMINISTRATIVE OUTREACH CLAIM SERVICES, IN AN AMOUNT NOT TO EXCEED $2,706,672, FROM JANUARY 1, 2000 THROUGH DECEMBER 31, 2002

RECOMMENDED: That The School Board of Miami-Dade County, Florida:

1. confirm a contractual agreement with The School Board of Hillsborough County, Florida, for Medicaid Administrative Outreach Claim Services, in an amount not to exceed $2,706,672, from January 1, 2000 through December 31, 2002; and
2. grant an exception to School Board Rule 6Gx13-3F-1.021.

C-13 REQUEST FOR AUTHORIZATION FOR THE SUPERINTENDENT TO ENTER INTO A COOPERATIVE AGREEMENT WITH THE MIAMI-DADE HOUSING AGENCY REGARDING ITS HOME OWNERSHIP OPPORTUNITY FOR PEOPLE EVERYWHERE (HOPE) VI REVITALIZATION PROGRAM FOR SCOTT/CARVER HOMES, FROM FEBRUARY 10, 2000 THROUGH FEBRUARY 9, 2001

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent to enter into a cooperative agreement with the Miami-Dade Housing Agency regarding its HOPE VI Revitalization Program for Scott/Carver Homes, from February 10,2000 through February 9, 2001. No local funds are required.

C-14 REQUEST FOR APPROVAL OF RESOLUTION NO. 00-05 OF THE SCHOOL BOARD OF MIAMI-DADE COUNTY, FLORIDA, ENDORSING MARCH 13-17, 2000, AS "YOUTH CRIME WATCH OF MIAMI-DADE COUNTY WEEK" IN ALL MIAMI-DADE COUNTY PUBLIC SCHOOLS

RECOMMENDED: That The School Board of Miami-Dade County, Florida:

1. approve the opening and closing hours of school (Pre-K-12) for the 2000-2001 school year; and
2. approve the exceptions to opening and closing hours of schools (Pre-K-12) for the 2000-2001 school year.

D-2 PROPOSED OPENING AND CLOSING TIMES FOR ELEMENTARY, MIDDLE AND SENIOR HIGH SCHOOLS FOR THE LAST THREE DAYS OF SCHOOL - JUNE 12,13 AND 14, 2000

RECOMMENDED: That The School Board of Miami-Dade County, Florida:

1. approve for June 12, 13 and 14, 2000, a reduction of the school day in all elementary schools to five hours and 15 minutes; in all middle schools to five hours and 30 minutes; and in all senior high schools to four hours and 30 minutes; and
2. Authorize the Superintendent to make any special adjustments necessary due to unique circumstances in particular schools.

D-3 REQUEST FOR AUTHORIZATION FOR MIAMI-DADE COUNTY PUBLIC SCHOOLS TO HOST THE EIGHTH ANNUAL MICROSOCIETY TRAINING CONFERENCE, JULY 16-20, 2000

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent to approve the Eighth Annual MicroSociety Training Conference venues of MAST Academy, Citrus Grove Elementary School and Citrus Grove Middle School.

D-4 REQUEST FOR AUTHORIZATION TO ENTER INTO A COOPERATIVE AGREEMENT WITH CAMILLUS HEALTH CONCERN, INC. TO PROVIDE MEDICAL SERVICES FROM FEBRUARY 10, 2000 THROUGH JUNE 30, 2001

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent to:

1. enter into a cooperative agreement with Camillus Health Concern, Inc. to provide medical services for students enrolled at Frederick Douglass Elementary School from February 10, 2000 through June 30, 2001; and
2. renew this agreement annually for up to two additional one-year periods.


There is no exchange of funds for these services.


D-5 REQUEST FOR AUTHORIZATION TO ENTER INTO A COOPERATIVE AGREEMENT WITH TOTAL FAMILY CARE CENTER, INC. TO PROVIDE MEDICAL SERVICES FROM FEBRUARY 10, 2000 THROUGH JUNE 30, 2001

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent to:

1. enter into a cooperative agreement with Total Family Care Center, Inc. to provide medical services for students enrolled at the Norland Triplex full service clinic from February 10, 2000 through June 30, 2001; and
2. renew this agreement annually for up to two additional one-year periods.


There is no exchange of funds for these services.

D-6 REQUEST FOR AUTHORIZATION TO LEASE FACILITIES FOR THE 1999-2000 SENIOR HIGH SCHOOL GRADUATION REHEARSALS AND EXERCISES

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent to:

1. enter into contracts with the following sites at a cost not to exceed $137,750 for senior high school graduation exercises, rehearsals, and associated costs of auxiliary services:
· Miami-Dade County Auditorium, June 13-16, 2000, at an estimated cost of $30,000:*
· Florida International University Golden Panther Arena and the Graham Center, June 13-16, 2000, at an estimated cost of $20,000;*
· Miami Beach Convention Center, June 13-15, 2000, at an estimated cost of $28,000;*
· Jackie Gleason Theater of the Performing Arts, June 13-16, 2000, at an estimated cost of $28,000;*
· Miami-Dade Community College-Kendall, June 13-16, 2000, at an estimated cost of $14,000;*
· Biltmore Hotel, June 13-15, 2000, at an estimated cost of $1,000;*
· Joseph Caleb Center, June 14-15, 2000, at an estimated cost of $500;*
· Gusman Center for the Performing Arts, June 13-15, 2000, at an estimated cost of $5,500;*
· UM/Knight Center-Ashe Auditorium, June 14, 2000, at an estimated cost of $750;*
· Rental of plants, pianos, crowd dividers, chairs, and other supplies at an estimated cost of $10,000;


*NOTE: Amount indicates estimated costs of rental and other auxiliary services related to the events, including, but not limited to, stagehands, electricians, ushers, police, audio-visual equipment, and parking.
2. enter into agreements for indemnification of the graduation sites listed above.
The appropriation for these items is included in the General Fund of the 1999-2000 Adopted Budget.

D-7 REQUEST AUTHORIZATION FOR STUDENTS TO TRAVEL OUT-OF-COUNTRY

RECOMMENDED: That The School Board of Miami-Dade County, Florida, approve the out-of-county field trip to Russia, Georgia, Belarus, and Azerbaijan, for students at Coral Gables Senior High, MAST Academy, Miami Senior High, Miami Springs Senior High, Miami Southridge Senior High, Southwest Miami Senior High, and Palm Springs Middle schools, March 29, 2000 through May 1, 2000.

D-8 REQUEST AUTHORIZATION FOR STUDENTS TO TRAVEL OUT-OF-COUNTRY

RECOMMENDED: That The School Board of Miami-Dade County, Florida, approve the out-of-country field trip to Paris, France for students at Design and Architecture Senior High School, March 30, 2000 through April 6, 2000.

D-9 REQUEST AUTHORIZATION FOR STUDENTS TO TRAVEL OUT-OF-COUNTRY

RECOMMENDED: That The School Board of Miami-Dade County, Florida, approve the out-of-country field trip to England and France for students at Miami Northwestern Senior High School, March 31, 2000 through April 9, 2000.

D-10 REQUEST AUTHORIZATION FOR STUDENTS TO TRAVEL OUT-OF-STATE

J-31 COMMISSIONING OF JOHNSON ASSOCIATES ARCHITECTS, INC. AS DESIGN CRITERIA PROFESSIONAL (DCP) FOR THE ADDITIONS, REMODELING & RENOVATIONS PROJECT AT NORTH TWIN LAKES ELEMENTARY SCHOOL, PROJECT # A-0773

RECOMMENDED: That The School Board of Miami-Dade County, Florida, commission the firm of Johnson Associates Architects, Inc. to prepare and administer the design criteria package for the additions, remodeling and renovations project at North Twin Lakes Elementary School for the following considerations:

1) a lump sum basic fee of $147,400 which represents 4.17% of the construction cost for services at North Twin Lakes Elementary School;

2) an additional lump sum fee not to exceed $8,000 was negotiated for traffic engineering study services;

3) at the Board’s option, the DCP will provide 90 site visits for a fee of $21,600 for the duration of the construction process;

4) the multiplier for hourly compensation was limited to 2.45 for extra work; and

5) the Project Architect will maintain at least a $1,000,000 blanket Professional Liability insurance policy.

J-32 APPROVAL OF AN EXCEPTION TO THE STANDARD SITE SIZE FOR LAWTON CHILES MIDDLE SCHOOL. SHADOWLAWN ELEMENTARY AND STATE SCHOOL "GGG"

RECOMMENDED: That The School Board of Miami-Dade County, Florida:

1) approve the exception to the existing site size for the educational facility referenced herein; and

2) authorize the Superintendent to submit to the Commissioner of Education at the Florida Department of Education) the Board-approved site exception for the educational facility referenced herein.

J-33 AUTHORIZATION FOR THE PAYMENT TO GE CAPITAL MODULAR FOR THE RENTAL OF MOBILE OFFICE AND INSTRUCTIONAL UNITS

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize payment to GE Capital Modular in an amount not to exceed $40,000 for the period March 1,2000 through August 31, 2000 for the utilization of 7 mobile office and instructional units.
