The following recommendations are made in accordance with Board Rules 6Gx13-44 1.16,6Gx13-4A-1.161, and the Miami-Dade County Public Schools/Dade County Schools. Administrators’ Association labor contract.

Considerations for reassignments of managerial exempt, professional, and technical personnel were predicated upon an intensive review of information by the appropriate administrators and the Superintendent of Schools.

RECOMMENDED:
That The School Board of Miami-Dade County, Florida, approve the recommendations as set forth above for appointments and lateral transfers, to be effective, November 18, 1999, or as soon thereafter as can be facilitated.

Six Design/Build contractors responded to the Request for Qualifications and three were selected for invitation to bid on the design criteria.

These projects include Minority Business Enterprise Subcontracting Assistance Levels.

RECOMMENDED:
That The School Board of Miami-Dade County, Florida award Project No. 0051 -BB, Design-Build, Remodeling Buildings 1 & 3, Mays Middle School, on the basis of Base Bid and Additive Alternate No. 1 in the amount of $1,455,953 to the low bidder:

J-30 AUTHORIZATION TO EXECUTE THE ABSOLUTE BILL OF SALE FOR THE CONVEYANCE OF WATER FACILITIES AT THE ADDITION EVERGLADES ELEMENTARY SCHOOL SITE IN FAVOR OF MIAMI-DADE COUNTY

Adequate fire protection and potable water services for the addition at the Everglades Elementary School site have been provided for by the installation water lines to the existing Miami-Dade Water and Sewer Department (MDWASD) utility infrastructure. The Board is now requested to execute the Absolute Bill of Sale for conveyance of constructed utility facilities to Miami-Dade County for operation and maintenance (see location map).

RECOMMENDED:
That The School Board of Miami-Dade County, Florida, authorize the Chair and Secretary to execute the Absolute Bill of Sale for water facilities at the addition at the Everglades Elementary School site in favor of Miami-Dade County.

ADDITIONS AND RENOVATIONS

This request for change in the construction contract includes ten items for the School Board’s confirmation. Attachment A presents a description of the items, the cause for each change, the effect upon the contract, and the action recommended to the Board. In order to prevent delay in the completion of the project, Staff authorized the Contractor to proceed with Items 1 through 10. These items are presented to the Board for confirmation and consideration as required by State Statute.

RECOMMENDED : That The School Board of Miami-Dade County, Florida, confirm Change Order No. 2 on Project No. A-0536, Additions and Renovations, Miami Gardens Elementary School, for an extra of $17,354.

This request for change in the construction contract includes five items for the School Board’s confirmation . Attachment A presents a description of the items, the cause for each change, the effect upon the contract, and the action recommended to the Board. In order to prevent delay in the completion of the project, Staff authorized the Design Builder to proceed with Items 1 through 5. These items are presented to the Board for confirmation as required by State Statute.

RECOMMENDED: That The School Board of Miami-Dade County, Florida, confirm Change Order No. 5 on Project No. KV-0060, Design-Build Renovations, Marjory Stoneman Elementary School, for an extra of $21,628.

The New World School of the Arts (NWSA) is a center of excellence for the performing and visual arts and offers academic and artistic studies for talented high school and college students. NWSA provides comprehensive programs to develop student artistic, creative, and academic achievements through a curriculum leading to a high school diploma, Associate of Arts degree, and a Bachelor of Fine Arts degree.

The following items are subject to approval by The School Board of Miami-Dade County, Florida:

1. Miami-Dade County Public Schools will finance the project by using a Certificate of Participation for the design/construction costs of the new facility;
2. the identified facilities list will meet the arts needs of both high school and college students as well as the academic needs of the high school program;

This open budgeted position was approved at the Board meeting of September 8, 1999, Agenda Item E-17.

RECOMMENDED:
That The School Board of Miami-Dade County, Florida, appoint Mr. Lawrence Rubio, currently a Senior Programmer Analyst I I, pay grade 38, to the open, budgeted position of EDP Audits Coordinator, pay grade 42, 12 months, and assign to Management and Compliance Audits, effective November 18,1999, or as soon thereafter as can be facilitated.

E-10 APPOINT AND ASSIGN:
AUDIT COORDINATOR III

The following recommendation is made in accordance with the Miami-Dade County Public Schools/Dade County Schools Administrators’ Association labor contract.

This open budgeted position was approved at the Board meeting of September 8, 1999, Agenda Item E-17.

RECOMMENDED:
That The School Board of Miami-Dade County, Florida, appoint Ms. Teresita M. Rodriguez, currently a Senior Auditor, pay grade 40, to the open, budgeted position of Audit Coordinator III, pay grade 42,12 months, and assign to Management and Compliance Audits, effective November 18, 1999, or as soon thereafter as can be facilitated.

E-9 APPOINT AND ASSIGN:
ELECTRONIC DATA PROCESSING (EDP) AUDITS SUPERVISOR

The following recommendation is made in accordance with the Miami-Dade County Public Schools/Dade County Schools Administrators’ Association labor contract.

This open budgeted position was approved at the Board meeting of September 8, 1999, Agenda Item E-17.

RECOMMENDED: That The School Board of Miami-Dade County, Florida, appoint Robert Vega to the open, budgeted position of EDP Audits Supervisor, pay grade 44,12 months, and assign to Management and Compliance Audits, effective November 18, 1999, or as soon thereafter as can be facilitated.

E-11 APPOINT AND ASSIGN:
STAFF AUDITOR (3)

The following recommendations are made in accordance with the Miami-Dade County Public Schools/Dade County Schools Administrators’ Association labor contract.

These open budgeted positions were approved at the Board meeting of September 8, 1999, Agenda Item E-17.

RECOMMENDED: That The School Board of Miami-Dade County, Florida, appoint:

1. Ms. Yvonne Barrios, to the open, budgeted position of Staff Auditor, pay grade 38, 12 months, and assign to Management and Compliance Audits, effective November 18, 1999, or as soon thereafter as can be facilitated;
2. Mr. Reginald Lafontant, to the open, budgeted position of Staff Auditor, pay grade 38, 12 months, and assign to Management and Compliance Audits, effective November 18, 1999, or as soon thereafter as can be facilitated; and
3. Ms. Liana Raquel, to the open, budgeted position of Staff Auditor, pay grade 38, 12 months, and assign to Management and Compliance Audits, effective November 18, 1999, or as soon thereafter as can be facilitated.


E-12 APPOINT AND ASSIGN:
PROJECT MANAGER,
SYSTEMS AND PROGRAMS

The following recommendation is made in accordance with the Miami-Dade County Public Schools/Dade County Schools Administrators’ Association labor contract.

This open budgeted position is due to the conversion of an open budgeted position.

RECOMMENDED: That The School Board of Miami-Dade County, Florida, appoint Kevin B. Gallagher, to the open, budgeted position of Project Manager, Systems and Programs, pay grade 43, 12 months, and assign to the Office of Information Technology, effective November 18, 1999, or as soon thereafter as can be facilitated.

G-78 AWARD OF BID #031-ZZII - COPYING EQUIPMENT, SERVICE AND SUPPLIES

The purpose of this bid, opened on October 5, 1999, is to establish a contract, to purchase or lease, at firm unit prices, quantities, as may be required, of copying equipment, maintenance and supplies, for various schools and departments. This is a term bid which states that the Board may purchase quantities, as may be required, at the unit price bid, but is not obligated to purchase any guaranteed amount. Of 129 vendors sent bid forms or card notices of this advertised bid, 20 _ responded, with 12 bids meeting specifications (including 6 bids meeting individual items specifications, but not all item specifications; not all vendors bid all items), 2 bids not meeting specifications and 6 no bids.

RECOMMENDED:
That The School Board of Miami-Dade County, Florida, AWARD Bid #031-ZZI 1 - COPYING EQUIPMENT, SERVICE AND SUPPLIES, to purchase or lease, at firm unit prices, quantities, as may be required, of copying equipment, maintenance and supplies, for various schools and departments, during the term of the bid, effective November 17, 1999, through October 31, 2000, as follows:
