RECOMMENDED: That The School Board of Miami-Dade County, Florida:

a) award Project No. A0748, Design-Build, State School "PPP", John A. Ferguson Senior High School, on the basis of Base Bid in the amount of $37,638,000 to the low bidder: Betancourt Castellon Associates, Inc., 7765 SW 87th Avenue, Suite 200, Miami, Florida 33173;
b) authorize payment for permit applications in the amount of $518,037.76 to Miami-Dade County in order to comply with the terms and conditions of the Department of Environmental Resource Management and the South Florida Water Management District/US Corps of Engineers Environmental Resources. Funds for this payment are budgeted in the Capital Outlay Program.

A-7 RESULTS OF THE 2001 LEGISLATIVE SESSION

A-8 REQUEST AUTHORIZATION FOR THE SUPERINTENDENT TO PARTICIPATE AS A CO-HOST WITH FLORIDA INTERNATIONAL UNIVERSITY'S CENTER FOR URBAN EDUCATION AND INNOVATION IN A NATIONAL REFORM SUMMIT ON JANUARY 17-18, 2002, AT THE BILTMORE HOTEL

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent to participate as a co-host with Florida International University's Center for Urban Education and Innovation in a National Reform Summit on January 17-18, 2002, at the Biltmore Hotel. There will be no additional costs to the district.

ACTION PROPOSED BY DR. PÉREZ: That The School Board of Miami-Dade County, Florida, establish a task force, to develop an instrument/procedure for the evaluation of the Superintendent of Schools.

1. present to the School Board an interim evaluation of the Comprehensive Reading Plan at its June 20, 2001 Board meeting; and
2. at the meeting, submit a three-year extension to the Comprehensive Reading Plan which reflects modifications based on the findings and recommendations of the interim evaluation.

B-7 REQUEST SUPERINTENDENT TO SUGGEST AMENDMENT OF SCHOOL BOARD RULE 6Gx13-5C-1.031, DRESS CODE

ACTION PROPOSED BY DR. KROP: That The School Board of Miami-Dade County, Florida, direct the Superintendent to bring to the Board meeting of June 20, 2001, suggested changes to Board Rule 6Gx13-5C-1.031 that would:

1. delete the requirement that ballots must be received from a majority of eligible parents, and
2. authorize a new vote to be conducted at schools which did not meet the majority requirement, at a time to be set by the Superintendent after the rule amendment becomes final.

B-8 A PROPOSAL THAT THE SCHOOL BOARD OF MIAMI-DADE COUNTY, FLORIDA, ABOLISH THE PRESENT COMMITTEE SYSTEM AND RESUME ITS FORMER PRACTICE OF MEETING TWICE MONTHLY

ACTION PROPOSED BY MS. KAPLAN: That The School Board of Miami-Dade County, Florida, abolish its present committee system and resume its former practice of meeting twice monthly.

C-1 REQUEST FOR AUTHORIZATION FOR THE SUPERINTENDENT OF HIS DESIGNEE TO ENTER INTO CONTRACTUAL AGREEMENTS WITH THE NATIONAL ACADEMY FOUNDATION, IN THE AMOUNT OF $5,000 EACH, FOR A TOTAL AMOUNT NOT TO EXCEED $10,000, TO ESTABLISH ACADEMIES OF INFORMATION TECHNOLOGY AT MIAMI CENTRAL AND MIAMI KILLIAN SENIOR HIGH SCHOOLS, BEGINNING WITH THE 2001-2002 SCHOOL YEAR

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent or his designee to enter into contractual agreements with the National Academy Foundation (NAF) to establish Academies of Information Technology at Miami Central and Miami Killian senor high schools beginning with the 2001-2002 school year. Said agreements would require a one-time NAF membership fee of $5,000 for each Academy of Information Technology, for a total amount not to exceed $10,000.

The appropriation for this item is included in General Fund of the 2000-2001 Adopted Budget.

C-2 REQUEST FOR SCHOOL BOARD APPROVAL OF THE CHARTER SCHOOL CONTRACTUAL AGREEMENT FOR DORAL ACADEMY HIGH SCHOOL FOR A PERIOD OF TEN YEARS COMMENCING ON THE FIRST DAY OF THE 2001-2002 SCHOOL YEAR

RECOMMENDED: That The School Board of Miami-Dade County, Florida, approve the charter school contractual agreement with Doral Academy High School, for a period of ten years, commencing on the first day of the 2001-2002 school year.

C-3 REQUEST FOR AUTHORIZATION FOR THE SUPERINTENDENT OF HIS DESIGNEE TO ENTER INTO A CONTRACTUAL AGREEMENT WITH THE UNIVERSITY OF MIAMI ROSENSTIEL SCHOOL OF MARINE AND ATMOSPHERIC SCIENCES (UM-RSMAS), IN AN AMOUNT NOT TO EXCEED $115,000, FOR THE FOURTH YEAR OF A FOUR-YEAR PROGRAM ENTITLED INSTAR, AN EIGHT-DAY SUMMER INSTITUTE FOR MIDDLE SCHOOL SCIENCE TEACHERS, FROM JUNE 18, 2001 THROUGH JUNE 27, 2001

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent or his designee to enter into a contractual agreement with the University of Miami Rosenstiel School of Marine and Atmospheric Sciences (UM-RSMAS), in an amount no to exceed $115,000, for the fourth year of a four-year program entitled INSTAR, an eight-day summer institute for middle school science teachers, from June 18, 2001 through June 27, 2001.

The appropriations for this item is included in the General Fund of the 2000-2001 Adopted Budget.

C-11 REQUEST AUTHORIZATION FOR THE SUPERINTENDENT OR HIS DESIGNEE TO ENTER INTO A CONTRACTUAL AGREEMENT WITH THE UNIVERSITY OF MIAMI-MAILMAN CENTER FOR CHILD DEVELOPMENT TO PROVIDE EDUCATIONAL SERVICES AND SPECIALIZED SPEECH TRAINING FOR DEAF STUDENTS, IN AN AMOUNT NOT TO EXCEED $494,550, FROM JULY 1, 2001 THROUGH JUNE 30, 2002

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent or his designee to enter into a contractual agreement with the University of Miami-Mailman Center for Child Development to provide educational services and specialized speech training for deaf students, in an amount not to exceed $494,550, from July 1, 2001 through June 30, 2002.

The appropriation for this item will be included in the 2001-2002 Tentative Budget to be recommended for adoption in July 2001.

C-12 REQUEST AUTHORIZATION FOR THE SUPERINTENDENT OR HIS DESIGNEE TO ENTER INTO CONTRACTUAL AGREEMENTS WITH THE FLORIDA INTERNATIONAL UNIVERSITY COLLEGE OR ENGINEERING AND DESIGN TO CONTINUE A PRE-APPRENTICE RESIDENTIAL ELECTRICAL WIRING PROGRAM AT MIAMI-DOUGLAS MACARTHUR SENIOR HIGH SCHOOL SOUTH, IN AN AMOUNT NOT TO EXCEED $33,042, AND BARBARA GOLEMAN SENIOR HIGH SCHOOL, IN AN AMOUNT NOT TO EXCEED $15,747, FOR A TOTAL AMOUNT NOT TO EXCEED $48, 789, FROM JULY 1, 2001 THROUGH JUNE 30, 2002

RECOMMENDED: That The School Board of Miami-Dade County, Florida:

1) authorize the or his designee to enter into contractual agreements with the Florida International University College of Engineering and Design to continue a Pre-apprentice Residential Electrical Wiring Program at Miami-Douglas MacArthur Senior High School South, at a cost not to exceed $33,042, and Barbara Goleman Senior High School, in an amount not to exceed $15,747, for a total amount no to exceed $48,789, from July 1, 2001 through June 30, 2002; and
2) grant an exception to Board Rule 6Gx13-3F-1.021.

The appropriation for this item will be included in the 2001-2002 Tentative Budget to be recommended for adoption in July 2001.

C-13 REQUEST FOR AUTHORIZATION FOR THE SUPERINTENDENT OR HIS DESIGNEE TO ENTER INTO A COOPERATIVE AGREEMENT WITH THE EARLY INTERVENTION PROGRAMS, PART C, TO PROVIDE SERVICES TO CHILDREN AGES BIRTH THROUGH TWO, FROM JULY 1, 2001 THROUGH JUNE 30, 2002

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent or his designee to enter into a cooperative agreement with the Early Intervention Programs, Part C, to provide services to children ages birth through two, from July 1, 2001 through June 30, 2002. No local funds are required.

C-14 REQUEST AUTHORIZATION FOR THE SUPERINTENDENT OR HIS DESIGNEE TO ENTER INTO CONTRACTUAL AGREEMENTS WITH THE DEPARTMENT OF HEALTH CHILDREN’S MEDICAL SERVICES EARLY INTERVENTION PROGRAMS AND FOUR COMMUNITY AGENCIES, TO OPERATE PROGRAMS FOR STUDENTS WITH DISABILITIES, BIRTH THROUGH AGE TWO, IN AN AMOUNT NOT TO EXCEED $4,572,918, FROM JULY 1, 2001 THROUGH JUNE 30, 2002

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent or his designee to enter into contractual agreements with the Department of Health Children’s Medical Services Early Intervention Programs and four community agencies to operate programs for students with disabilities, birth through age two, in an amount not to exceed $4,572,918, from July 1, 2001 through June 30, 2002, as follows:

1) Association for Retarded Citizens of South Florida will serve a maximum of 122 students, in an amount not to exceed $1,331,277;
2) Easter Seals Miami-Dade, Inc., will serve a maximum of 131 students, in an amount not to exceed $1,417,736;
3) University of Miami, Debbie Institute will serve a maximum of 92 students, in an amount not to exceed $846,952; and
4) United Cerebral Palsy Association of Miami,, Inc., will serve a maximum of 103 students, in an amount not to exceed $976,953.

The appropriation for this item will be included in the 2001-2002 Tentative Budget to be recommended for adoption in July 2001.

C-15 REQUEST AUTHORIZATION FOR THE SUPERINTENDENT OR HIS DESIGNEE TO ENTER INTO A CONTRACTUAL AGREEMENT WITH THE UNIVERSITY OF MIAMI AND THE DEPARTMENT OF HEALTH CHILDREN’S MEDICAL SERVICES EARLY INTERVENTION PROGRAMS TO PROVIDE EDUCATIONAL SERVICES TO 120 HANDICAPPED COCAINE-EXPOSED STUDENTS, BIRTH THROUGH AGE TWO, IN AN AMOUNT NOT TO EXCEED $1,139,183, FROM JULY 1, 2001 THROUGH JUNE 30,2002

RECOMMENDED: That The School Board of Miami-Dade County, Florida, authorize the Superintendent or his designee to enter into a contractual agreement with the University of Miami and the department of Health Children’s Medical Services Early Intervention Programs to provide educational services to 120 handicapped cocaine-exposed students birth through age two, in an amount not to exceed $1,139,183, from July 1, 2001 through June 30,2002.

The appropriation for this item will be included in the 2001-2002 Tentative Budget to be recommended for adoption in July 2001.

C-16 REQUEST AUTHORIZATION FOR THE SUPERINTENDENT OR HIS DESIGNEE TO ENTER INTO A CONTRACTUAL AGREEMENT WITH HOWARD A. ENGLE, M.D., TO PROVIDE MEDICAL CONSULTATIVE SERVICES, IN AN AMOUNT NOT TO EXCEED $75,000, FROM JULY 1, 2001 THROUGH JUNE 30, 2002

RECOMMENDED: That The School Board of Miami-Dade County, Florida:

1. authorize the Superintendent or his designee to enter into a contractual agreement with Howard A. Engle, M.D., to provide medical consultative services, in an amount not to exceed $75,000, from July 1, 2001 through June 30, 2002; and
2. grant an exception to School Board Rule 6Gx13-3F-1.021.

RECOMMENDED: That The School Board of Miami-Dade County, Florida, commission the firm of A.D.A. Engineering, Inc., to perform civil engineering services for a retention pond/lake project at Doral Middle School for the following considerations;

1) a lump sum basic fee of $59,725, which represents approximately 9.95% of the estimated construction cost of $600,000;
2) additionally, at the Board's option, the firm will provide 30 site visits for a fee of $6,600, for the duration of the construction process;
3) the multiplier for hourly compensation was limited to 2.45 for extra work; and
4) the firm will maintain a project-specific Professional Liability insurance policy of at least $500,000, for which the firm will be reimbursed up to a maximum of $5,435.

J-41 AMENDMENT TO AGREEMENT WITH PJB ASSOCIATES, P.A. TO PERFORM DESIGN CRITERIA PROFESSIONAL SERVICES FOR A NEW MIDDLE LEARNING CENTER (STATE SCHOOL "TT") TO CONVERT STATE SCHOOL "C", A NEW ELEMENTARY SCHOOL, TO A K-8 FACILITY, PROJECT #A-0715

RECOMMENDED: That The School Board of Miami-Dade County, Florida amend the agreement with PJB Associates, P.A. to perform design criteria professional services for a new middle learning center (State School "TT") to convert State School "C", a new elementary school, to a K-8 facility for the following considerations:

1) a lump sum basic fee of $181,750, which represents approximately 3.64% of the construction cost;
2) additional lump sum fees were negotiated for the following consulting services: a) $8,275 for providing traffic engineering studies; and $5,500 for providing food service consulting services,
3) at the Board's option, the DCP will provide 93 site visits for a fee of $23,300 for the duration of the construction process,
4) the multiplier for hourly compensation was limited to 2.45 for extra work; and
5) the Project Architect will maintain at least a $1,000,000 project-specific Professional Liability insurance policy, for which the DCP will be reimbursed up to a maximum of $13,000.
